The only things which grow back are things which keep growing. Fingernails and hair for example. Foreskin neither keeps on growing, nor does it grow in cycles like eyelashes for example. It is a much more intricate part of the anatomy than those things and contains thousands of nerve endings. Some animals like salamanders and spiders can regrow their limbs. It's very complicated to explain but they are able to generate a particular type of protein which their genes allow them to produce which helps the process along, a process in which the details are far too arcane for me to explain. To simplify it (for both our sakes) humans haven't evolved this ability and strangely enough, the salamander genome is actually larger than the human genome which is what has allowed them to develop such astounding natural abilities. Even if we did share this ability with salamanders, I'm not sure it would extend (no pun intended) to penises.

They don't so much fall off but are outgrown by the rest of the body. It basically ends up as your tail bone (coccyx). However, some people actually develop a very small vestigial tail. A clue to our evolutionary history.
