Is "are they real" the ultimate boob compliment?

Not really because usually it is easy to tell. Implants have several tell tale signs. They are always overly firm, distinctively shaped, too big a gap between them and there is always a scar at or near the bottom crease. "Real ones have none of those and are soft and supple throughout. They bounce differently too and in fact many implants don't even bounce or jiggle at all. I suppose if one wants to express a compliment by implying that they are so wonderful and perfect that they couldn't possibly be real one could but only if one does not imply they are implants but rather that the guy must be dreaming or that they must have come about via magic. I once gave the mother of a girl I dated a thank-you note for making a beautiful girl with her contribution to her DNA. Her daughter clarified by explaining that I liked the body her mother created. I think there are a lot of ways a guy can be creative when complimenting a woman's body parts without comparing her to two chunks of plastic.

I'd say it's based on how it's said. Like, if it was made from disgust or jealousy, obviously not. But if it's from curiosity, maybe amazement, then I'd say so.

No. The ultimate boob compliment (in my opinion) is when you don't need her to take her bra off. (Though, according to some, it can also be a backhanded compliment, depending on the level of gravity.)
