You volunteer as a befriender (someone who visits and chats with people who are terminal ill or elderly). One of the patients you see is a woman in her early 50s who is terninall ill, but lost her husband a couple of years ago in an accident and she is lonely. You've been visiting her for about six months but it's looking like she is beginning to come to the end of her time, but is still quite agile. You've made quite a connection with her and can easily talk for hours, she's advised you on problens and helped you with advice, basically she's helped you as much as you've helped her. She still looking quite good for her age and considering the illness, but has recently lost her teeth because of her illness, and has got dentures now.
So, one afternoon you are talking in her lounge and she is talking about her late husband and reminiscing, and she gets on to the subject of their sex life. She confesses to you that she has never sucked a penis before as her husband was a bit proper and wasn't having any of that funny business, and regrets never having tried it before she dies. She looks quite sad about it.

So there you are, you know it's morally wrong but you also know she is of sound mind, and the 2 of you are quite close, and you really feel for her. You also know nobody would ever find out.

It’s wrong on a number of levels and very much a reportable issue.

in any role where you are acting in a role of trust, support, helping someone etc, there is a duty of care and of not exploiting the emotional bond that can build up.

in this case it was an emotionally vulnerable person.

The real world problem with this, is the person finding out they can get things, gifts, sexual favours, then moves on an exploits other people, each time often worse than the last.

in that position, not only would I say no, but I would also report it and ensure a female carer was assigned.

I've spent enough time working with vulnerable adults to know how bad this hypothetical question is.

The other side with these things, she tries it with next carer who objects and she says ‘but John used to…..’

I am in agreement with you, I've worked on and off in the medical profession over the years and ethics is something I take very seriously. While the person was not a paid medical person or a carer, it doesn't take away from the fact that he was in a position of trust.

There are two directions, and both have been addressed. @chrismaster69 explained the moral responsibility of a caregiver, and also the legal side. @loves2learn explained a different moral condition. One where the caregiver has really become close with the person he is helping, and the woman has really become close with him and is able to be completely open. She wanted very badly to do a sexual act she had never done, and the caregiver, being a male, could give it to her. It was actually a loving gesture to honor her wish. I am sure both of them had very strong feelings before, during, and after the act. The law would likely look at it as he took advantage of her. But in reality, he gave her something special without hurting her.
