The most painful thing a person can experience is burning alive, then childbirth if no pain management is involved particularly the first child. Followed very closely by a male passing a kidney stone larger that 3-4 mm and a blow to the testicles which people dont take into consideration that permanent pain is possible from a testicular trauma.
You also have to take into consideration the fact that most women have a higher pain tolerance than males.

I can only describe to a woman what I feel when being hit hard in the nuts. If you get hit hard enough It feels like your kidneys are about to explode inside of you while at the same time you have to vomit up your own intestines while also being out of breath. I don't know what menstrual cycles are like but feel free to share your own experiences under my comment.

I imagine a blow to the testicles to be equal to birth contractions, which are far worse than menstrual pain. Of course, the amount of pain is different for every woman, some experience immense pain during their periods, so I can only speak from my own experience.

I'm sure that a hit to the privates will hurt more and it will be a much sharper pain but it will last much shorter than period cramps. These things can last for days up to a week for some women.

I think squeezing and holding the pressure on his balls would hurt more than a blow. But that's what mensural cramps feel like. The pains are probably the same.
