The letter refers to the difference between your bust measurement and your underbust measurement. DD is a 5 inch difference and contrary to what society tells you, this is not large. Also, DD isn't static either - a DD on a 26 inch underbust (26 band, 26DD) will be much smaller than a DD on a 40 inch underbust (40 band, 40DD). For example, Ariana Grande is roughly a 26DD, and I don't think anyone would call her large-chested.

The reason you think 'normal small women' wear a 32, not a 28 is because of something called the +4 method. The +4 method is an incorrect measuring method that many stores use. It tells women to add 4 inches to their underbust measurement to get their band size, this is incorrect. If your underbust measures 28 inches, your band size is 28. But so many women are told to add 4 inches to this, so the women who need 28 bands are smaller are stuck in 32 bands because shops don't offer anything smaller - because of the +4 method. Their smallest size should be 28, but because of the +4 method, their smallest size is 32 - which is actually more on the medium size when correctly fitted. The women you consider 'normal small women' are mostly 28 bands or smaller - sorry to burst your bubble. Cup size is calculated from the difference in inches between your underbust and bust measurement. If you're adding inches to your underbust measurement, you're narrowing the difference between the measurements (cup size) and therefore will be in a smaller cup size. When your cups are too small, this can make even a way too loose band feel tight because of the extra strain that is put on the band from your boobs trying to accommodate more space in the too-small cups. This is part of the reason as to why the +4 method has lasted this long and is still so widely used. People don't realize how badly fitted their bras are.

Sydney has a small frame, probably a 24-26 inch waist meaning her underbust would be around 26-28 inches, so somewhere around a 26GG/28G - because a D cup on her frame would be very small.
