2. The name is misleading for several reasons. Nothing actually turns blue, but "blue" was a common slang term 100 years ago for both sexual content (comedians who told "dirty jokes" were referred to as "working blue" and early porn was referred to as "blue movies") and for "shameful pain or injuries"). And the "balls" obviously refers to the testicles, but it's not actually testicles that the pain originates from, but rather the prostate gland, which is close by.

The prostate gland both creates semen (which doesn't yet contain sperm at this point - sperm gets injected into the semen during ejaculation) and stores it. Normally the prostate works at a leisurely pace to fill the "tank" with semen until it's maybe 80% full, but when a man becomes aroused, the prostates starts spooling up to make semen at a much faster rate, and the more aroused the man gets, the faster the prostate tries to work. The prostate fills to 100% and then keeps going, so that there is pressure starting to build. This would normally be painful immediately, but endorphins tend to mask this pain, so you just have the pressure that's building sexual tension, which is normally a good thing, as that sexual tension is useful in achieving orgasm. The longer the man is kept in an aroused state without ejaculating, the more the pressure (and tension) builds, expecting a release (ejaculation).

But if the release never comes - say, after a 20 minute intense make-out session - then the endorphins quickly wear off and suddenly the pressure and swelling of the prostate is causing pain that the guy can feel - it's like a combination of a bruise and a cramp - and everything in the lower crotch area is extremely sensitive to squeezing or the tiniest of impacts, because it's swollen and under pressure.
