I've never heard it used towards a gay person. I've only heard it used as an insult towards another guy for doing something out of character for that particular guy.
As a straight guy I've never had a problem with gay guys honestly. I don't really understand homophobia.
I have heard straight women make hateful remarks towards gay males before, but I think they feel threatened by it. Obviously gay men don't need women and that may annoy them?
