  • The best way to explain blue balls to a woman is that they are similar to mild menstrual cramps. Except that where a woman's cramps end with the passage of time, blue balls can be alleviated by ejaculating.

    The fact that sexual pleasure ends the condition is part of the reason women disbelieve in it. It goes against their experience of cramps.

    The cause of both cramps and blue balls are the same -- restriction of blood flow. In the case of cramps, contraction of the uterus may press on nearby veins and arteries and temporarily cut of oxygen supply to the muscle tissue of the uterus. Ending the period is the solution to the problem.

    In the man's case, the blood vessel constriction that enables erection also causes blood accumulation in the scrotum and testes. Blood is coming in at a faster rate than it is leaving. That causes swelling and pain. Sexual release is what fixes this because after orgasm the erection subsides, thus returning normal blood flow. The problem is that masturbating when your genitals are already sore isn't very much fun (unless you're a masochist).

    • I think the pain is akin to menstrual cramps. Not all girls experience menstrual cramps. Those who do, the pain can be mild or debilitating. My bb pain starts as a mild pain on my right testicle (usually day 3), it would then extend to my right waistline. From day 5 to 10, the pain becomes uncomfortable but without any noticeable impact on my mobility or day-to-day functioning. From day 11, the pain extends to my left testicle and left waistline, and becomes constant and debilitating. To seat down, I'd need to do it slowly. The twitching or piking pain affects my focus. Day 14, I take painkillers, ibuprofen for two straight days, which really works for me. Is my bb serious? Yes. Can I handle it? Yes. Is it life-threatening? No. Not all guys experience blue balls. My BB only arises when I have sustained erection, or multiple erections in a day, week or month, without ejaculation.
