It differs from men to men, some come more and some come less, in general people seek medical help if there is (almost) no semen coming out. If he has no other complaints and everything else works as intended I would not seek medical help either.

Testicles can be different in size, although it should not be half/double differences, or sudden changes like swelling / pain. There is no link though between testicle size and volume of the ejaculation, most of it is made anyway inside the body (prostate, etc).
