I have to see a dermatologist often for full body scans. My dermatologist is a female and I’ve seen her for years now. She’s very thorough and not shy about examining my penis and scrotum. Also, why did I have that reaction after years of seeing her?

Doctors are trained to deal with this. A reaction like this is not controllable, despite some claims by ignorant people. If you are going to blow, just warn her that you need a minute and move her away from you. Trust me, it's awkward for her too.
