im a stripper. I've seen it all. where I work there definitely is touching. lapdances and things like that so basically girls are grinding and rubbing all over your boyfriend. in the v.i.p. I've definitely seen (and been a part of) sexual activity, but that's only because the men there have tons of cash and its a free for all.
whether or not they cheat is circumstantial to your relationship. some women are okay with men going to strip clubs and know that there's touching and lap dances and things like that. in those cases, it isn't cheating. if you tell him that you don't approve of that though, and he goes anyway, yes there's a chance he is cheating and not telling you about it. some men do come just to watch though so not all can afford the real action that goes on in vip or private shows and not all places allow touching and lap dances but many do.
men lie about it all the time though, yes. I've had customers who even told me their girlfriends or wives didn't know they were there while I'm naked on top of them in a private show. things like this have been going on for centuries though, its nothing new.

The vast majority are law abiding establishments. There's usually an asshole who watches you like a hawk and he'll throw you out if you touch the girl.
You get there, over pay just to walk through the door. Then they overcharge you for drinks. Girls could be pole dancing. You can sit and watch. Girls walk around between dances and offer a lapdance. Basically you sit down on your hands and they sit on your lap and dance for one song.
You're not allowed to wear shorts or any sheer clothing. Most places are topless only. The ones that are full nude often don't serve alcohol.

Generally, if your man's going to a strip club, you should be worried. A lot of activity such as this does happen. There's almost always touching unless the guy is strictly going there to view. If a guy is going to a strip club more and more often then the chances of these activities, opposed to just watching, tend to rise.

No, he's fundamentally wrong. What you've been told about no touching is essentially correct. If a girl is giving the guy a lap dance, he's not allowed to touch her. However, many of the girls there could very well also be active escorts. It's not like you can ask, "Are you an escort?". She'll likely get pissed and walk away. However, if you make friends, buy her drinks, the chemistry is there the guy just might find out what he could get in the 'VIP room'. It's very expensive but...

Well it wouldn't be a stretch to imagine stuff like that happening at a strip club. There are certainly ones that have a strict no-touch policy. But again, it wouldn't surprise me if stuff does go on.
