When a man is aroused, one of the many things that happens is that the prostate gland starts working overtime producing semen, in anticipation of an impending ejaculation. This causes a bit of a feeling of pressure in the prostate, which is located just behind the scrotum. That feeling of pressure is one of the things that adds to the "urge" or "drive" to have sex and ejaculate.
When a guy is *really* turned on and amped up, and those hormones are raging around, this pressure can grow to painful levels, but with the endorphins from arousal suppressing most of that pain, the guy probably won't really notice it too much. UNTIL... the sexual activity is broken off. The endorphins fade, and suddenly the guy is in pain, a pain not very different from a strike to the testicles. And at that point, the guy either has to wait up to several hours for the pressure to dissipate, or he's got to ejaculate. It isn't hard to figure out which the guy will prefer.
Now, this doesn't happen every time; a guy can be aroused without being SUPER-aroused and without that huge over-pressure feeling, but it isn't uncommon, especially with younger guys, who may have a long make-out/feel-up session with no "finish", and whose hormones are already working overtime.
But when it does happen, the guy is going to want/need a release. Obviously, masturbation would work just as well as any other method, but many guys will try to get their girl to help them out in one way or another, and I suppose it's up to the girl to decide what to do.
But the condition is very real, if slightly misnamed; it is the prostate gland, not the balls, that hurt, and the balls don't turn blue, it just feels like it.
It doesn't feel like getting kicked in the balls, it feels like the feeling a few moments AFTER you get kicked in the balls.
Taking a shot to the nuts results in sharp, excruciating kind of pain. After a few moments that subsides and gives way to a dull, aching feeling which is precisely what "blue balls" feels like. Blue balls isn't horribly painful but it is very uncomfortable. It's unpleasant but it's bearable.
Little known fact: Girls get a similar condition when we're aroused and don't finish. If you think about it, you've felt it. We're just used to feeling discomfort in our lady parts since it happens so often. link
As far as I can tell (not being a guy), in extreme situations it can be very painful, but it doesn't often get to that point.

I think its just a psychological think. I haven't come in 145 days with permanent teasing. It sucks yes, but it never hurts :)
I got much experiance in orgasm denial, so I had to suffer "blue balls" much in my live. It just makes you horny and reaaally want to orgasm. If you haven't ate chocolate for some weeks and someone is showing you choclate, you would also say that you get hurt when you don't eat it right now! ;)

It depends. If he's been teased or horny often without release it can be extremely painful and take a while to go away. Sometimes days. But if he's just been busy with no teasing or sexual acts it probably wouldn't feel bad for a long time. It does exist and I imagine differs from guy to guy but have heard of it being used as an excuse. But it is very real.
I can only assume the guys here who don't seem to know what the f*** they're talking about with Blue Balls, have never had a girl friend.

It's uncomfortable, not crippling. It can be mild to quite uncomfortable, but never to the point of debilitating. Maybe I'm just weird, but it seems like most of the pain comes from blue balls touching stuff, so the last thing I want when I've got blue balls is to get head. I just want them left alone at that point.
