My current Girlfriend is the only person I've ever done it with in a relationship, and it was very much her idea. I even resisted it for some time, because I didn't want it to come between us, but she eventually convinced me that it was what she wanted, and we talked about how she would feel about various things that might be involved. I didn't agree until I was convinced she could handle it.
For the most part, threesomes are best left to casual relationships where there are no feelings involved. It's very much the exception that you can have feelings for someone and not have those get messed up by a threesome; it really requires that both people in the couple are honest about what's going to happen, and how they might feel about that.
Obviously, actually doing it is hot and a lot of fun. In my case, my Girlfriend is bi, so she wanted to be with another girl, which was the only way I'd have been interested anyway. If a MMF was important to her, that could have been a problem for me.
