Is it a bad idea to tell someone they've done nothing wrong?

So whenever I get upset with someone over something, usually people that I care about like family or close friends, I don't tell them what's wrong. Like, the person could flat out ask me why I'm upset or if they did something wrong and I say no or that I don't think they did. I don't want to hurt their feelings or have them be upset with me. So my question is, is that bad? Should I let them know that they've hurt me so they don't do it again? I'm just always afraid that they'll think I'm overreacting or that they'll be offended.
