I think 99% of the time nobody questions the 'I'm fine' cause it's convincing. Besides, it's not always that deep. If someone is not fine they might just had a bad das and not a bad life. And it's about the intentions. People usually say it to safe time or they don't want to bother the other Person. So it's not said with bad intentions.
You've got it ass backwards, every guy alive knows the real meaning of "I'm fine". That's destructive thinking to think that telling then would bother them, they wouldn't have asked if they were okay in the first place if they weren't willing to be there for them and listen😒
Don't people in the US say it as sort of a greeting? Like the cashier could say it to you.
