It's used to mean family is more important than friendships because it's a stronger bond, but ironically the original saying it's taken from means the opposite:
'the blood of the covenant is thicker than the water of the womb'
Family is the most important thing. And a family... could be like your best friend who you consider a sister, they are always there for you, through thick and thin.
