Apparently, I am absolutely awful at telling the difference between just hanging out and a date. (According to my friends I've been on 3 dates this year, but in my eyes we were just hanging out...) How can one tell the difference between just hanging out and an actual date?

Ruling out particularly romantic activities, generally it's a difference of intent. If you are simply enjoying the others' company without regard for how this might affect how you regard each other in the future, then you are hanging out. If you are checking him out to see if there is potential for a relationship (in addition to whatever else you're doing), then it's a date. Note that an outing can be a date for one and hanging out for the other.0
