I naturally have a REALLY bad slouch. Sometimes I can feel it and straighten up, but I quickly fall back into slouching again. How do I fix it for good?

I think you need to be aware of it and always straighten up. But honestly no one is straight and direct then we would all be like trees or buldings erect and stiff. maybe your making it seem a big deal in your head or you can still get a chiropractor to work on your spine :D0
Well you just have to stand straight and remember to stand straight.. and practice it...
In some time it will become a habit.. :-)0
bring up ur core strength and try deadlifts and upper back muscles, they help support good posture0
