How do you stay motivated while getting fit?

Looking for tips to stay motivated long term. I have bounced up and down on weight through various programs and workout routines. But I have never succeeded in changing over to a healthier lifestyle. Can anyone share a success story about changing this permanently. For context I have about 40 lbs to lose and want to stay around that range moving forward.
