Excellent response Jim! You've traveled to over 70 countries as I can recall. How many of them shitholes? I think you said Cambodia was awful as well?
I had a bit of a think about this, before answering. Cambodia and Lesotho are the two visibly poorest countries I've ever visited, and each of them does have shithole characteristics, such as police corruption etc. I wouldn't want to get ill, or in trouble with the law, in either. I didn't like all the kids working to sell you shit, when they should have been in school, in Cambodia. I really didn't like the two drunks having a fight, involving a broken bottle, in the car park of the Lesotho border post, at all. Having said that, I didn't feel markedly unsafe, or like an obvious target, in either. Pestering was not constant, or persistent, and you weren't expecting bombs to start going off. So, probably the worst was Turkey. People who spend two weeks in a resort, on the Turquoise Coast, won't understand this, but Istanbul left me checking where the exits were every time I went into a restaurant, or other enclosed space, and you had the feeling, constantly, that anything could happen, without warning. But I do make it a general rule to avoid true shitholes, so that's the closest I've come, I think?
