As in; would you accommodate one or two of their physical preferences if you didn't have them, or pick up/drop a habit (smoking, drinking, exercise, etc. )? Assume this is someone you envision spending your entire life with.

Personally, I'd go with the 2nd option. I would, if they were willing to do the same for me:
I'd be willing to accommodate physical preferences to the extent that they are within my ability, and aren't extreme (like plastic surgery, etc.). I would only be willing to pick up habits I consider good e. g. I'd quite drinking.
