It is. It's a sensitive topic and you should never ask personal questions or comment about anyone's body even if they look pregnant. People gain weight for multiple reasons maybe she's going through a tough period. Maybe she's trying to conceive but having fertility issues. You never know what someone is going through and is best to not assume/ask anything.

It is. A) If they haven’t told you they’re pregnant they don’t want you to know (yet) and it’s non of your business. B) If she’s not pregnant then you’ve just let her know you think she looks pregnant. It’s like walking up to someone and saying ”you’re fat” or ”you have a huge stomach”. Leave peoples bodies and private business alone.
As a guy we don't mean it to sound rude, but I've been told not to say it in future!!

Yes. If the woman is not pregnant. I have a friend who was asked that question before, she actually have PCOS. Now every time we go to a bathroom mirror, she would look at herself and cry. Mind you, she was confident before. 🙃
Maybe the question would be okay for medical, or emergency situations.
Perhaps in the right context of things it could be deemed acceptable to ask, otherwise it's extremely inappropriate.
