Short answer: in order to obtain a work visa for the United States he must first find an employer who will petition the Department of State to consider him for the visa. The job offer must be specific, and not simply general employment. The employer must also explain that there is a shortage of American citizens to fill that particular job.
The last time I checked, there was no shortage of, or tremendous demand for, curtain and tile designers here in the USA. In any case, if the man's English isn't good enough to ask this question himself, he won't stand a chance in an American workplace.

Build his portfolio. Get some buisness contacts if possible. Send free samples of work to interested people. If he is competetive enough and offers value at a good price for his skills he might find a way.

Sounds just about impossible unless he starts his own business. This is a question you might want to ask on website for expats in an individual country because the answers will vary.
