It hurts like hell while it is possible to remain standing you will most defiantly be brought to your knees at the very least. And if she actually hits the sack you will defiantly fall to the ground.

Doesn't matter how big someone is, everyone has weak points. The groin just so happens to be guys number one weakpoint. If a girl precisely and with enough force hit a guy in the nuts, she could easily drop him it doesn't matter if it's Dwayne the rock Johnson, he will drop if he gets hit there.
I have no idea but if your balls are hit hard enough it will release a deadly amount of adrenaline that would kill you instantly!
