I got 88 and here's how I interpreted it.
In the first row, there's three of the exact same shape, so you divide 45 by 3 and you can assume that the shape represents 15. This is the part that everyone seems capable of understanding...
For the second row, using what we know about the grey shape, we subtract 15 from 23 to get 8. So then seeing as each of the banana bunches in this picture have 4, we can come to the conclusion that each individual banana is equivalent to 1.
Knowing what we figured out in the previous row, we subtract 4 from ten and get 6... There's two clocks that each have a 3 on them, and 3 plus 3 equals 6.
The last row is where we apply all the knowledge we've gained!
Notice the clock is pointing to 2, representing a value of 2. And notice there are now 3 bananas! So 2, 3 and 3. And knowing that the grey shape is equivalent to 15... And so each shape represents a different value. The number of sides equals the number value it is.
Basically, at the end, we get...
2 + 3 + 3 x (6 + 5)
Which is... 8 x 11 which is 88!

It gets 45 from the 15 points from the hexagon, pentagon and square combined.
23-15= 8. 8/2 = 4. Each banana equals 4. If you look closely on each banana, the banana on the last line has 3 unlike 4, the clock is at 2 o'clock instead of 3 o'clock and there's 11 points in the shape because its missing a hexagon.
2+3+3×11 = 38
