He had been calling me his girlfriend. Then he decided to call me his "lady". Made it sound like it was more than girlfriend, but I wonder if it is a step up or a step down.
It's just another affectionate form of describing your relationship. My fiance calls me 'his woman' when talking to friends on the phone or whatever. I think it doesn't necessarily mean a step anywhere, but if I had to choose one or the other, I'd say it's a step up. "His lady" or "his woman" are more possessive sounding that girlfriend. It's a good thing :)

Not at that age yet, but I would assume it's a step up. It means that you're not some girl, but a mature woman that he's with. To call you a lady may also imply that you're someone with class.
