Well, if you say "do you want to go out with me," there is a slight possibility that she might take it as "do you wanna grab lunch or something." I mean, I think most girls in middle school and high school would understand that you want to make them your girlfriend, but just to be safe, choose a line that actually includes the word "girlfriend" in it.
I'm only saying this because I had the misfortune of not completely understanding a guy once. I totally would have been his girlfriend too, only I was too dumb to realize that that is what he wanted right at that moment. I think I was just incredibly nervous, so my thinking was shutting down or something... so if she is one of these girls, make it clear what you mean to say so that you don't call her your girlfriend one day and she acts totally surprised because she didn't realize that she actually was your girlfriend :S yeahh... that's an awkward situation...
I've had so many awkward situations... I feel like they should all be done by now, but every time I find another guy at my door, I just got to do something weird and oh, well I'll stop babbling now... try this:
"Hey there ________, so I was wondering, would you like to be my girlfriend?" That's right, short, sweet, and simple...
And if she gets all red in the face and her knees start buckling and she says "I dunno, do you want to be my boyfriend?" Please just go with it... go with it for this opinion sharer here. Thank you.
When my boyfriend asked he straight up said, "Would you be my girlfriend?"
Right to the point. I mean, I'm pretty sure he knew he wasn't getting rejected seeing as he asked me while we were on a date that I asked him on. :p
Make her laugh first ask if she likes stick of bread and then tell her something like there are six billion people in the world that mean there are twelve billion hands to hold and yet only you I want... then ask her out :)

"we can hangout sometimes"
or simply
"wanna go out cz its 'your cheesy line'"
