How to learn a foreign language to a conversational level on your own?

Maybe for you-I live most of my life now outside of the US and I have found that, even if I memorized all of the nouns and verbs and cases and declensions of a particular language (Estonian in my case) there was absolutely, positively no way I could be conversant (I am ) in a language without have to speak it day after day, month after month in-country. There is a HUGE difference between knowing how to say "I want a glass of milk" and actually having to get all of the consonants and vowels to come out of my hung-over mouth to a waitress at a restaurant in the morning before I actually can get a glass of milk. :)
But that's just me-and there are probably more linguistically talented people out there :)
