It. I never watched it but I saw a copy of it in a grab 3 for 2 section as a kid and my older sister is deathly afraid of clowns.
In an act of kindness I gave it to her as a gift. Maybe it was Halloween? Many minutes of screaming occured. I then, as a good brother, planted it at various unexpected places throughout the next week. After which it was utterly destroyed by her. But the cover alone did more than any movie since has. So there's the winner for me. No idea if the movie is any good though.

The Thing. Practical effects that hold 40 years later, and a monster that isn't just deadly, but creates suspicion and paranoia. When you can only trust yourself within a group in isolation, that's scary.
Cloverfield, which to me is more Lovecraft than typical monster flick.
For those who don't count that as horror, Invasion of the Body Snatchers (1978)

The Grudge is the only one that really freaked me out. It's based off a Japanese superstitious belief.

The original Thing movie is awesome, the ring was ok, the japanese version of The Grudge is scarrier than the Sarah Michelle Gellar version.
