I'd unfortunately be willing to bet that such behavior is the result of some form of abuse. I'd talk to her about it and be damn sure that her parents are not aware that you did. Because the abuse that caused such a behavior might be happening at home. So her parents finding out that you talked to her might inspire more of it.

If this is actually a serious question and not trolling, I've only seen kids and teens urinate or defecate in random places outside of the bathroom when there is a history of sexual abuse along with pretty serious mental health issues. If you have any behavioral health facilities where you live, I would take her for an assessment and to get mental health treatment.

Not trolling at all. This is a big problem and she won’t stop drinking caffeine. My whole family is worrying. My sister is very secretive about their home life. Who could be sexually abusing her?

Statistically, sexual abuse is usually perpetrated by someone the victim knows such as a family member or a close family friend. I don't know but either way, it's sad and she needs some help.

I don't know what could be going on. She may be afraid to talk about it or feel too embarrassed or ashamed. It may help for her to have someone outside of the family to talk to like a therapist.

That depends on where you live. In some places, teens over a certain age can legally access mental health services on their own without a parents permission but there are some places where parental consent is still required. You will just need to research the laws in your area.

Our strange actions like this in normal thing for her? There's definitely something wrong with her. Once you've been potty trained you know to use the restroom. She should probably be taken to the family physician and they will refer her to wherever she needs to go

It's in reference to Regan from The Exorcist. But in more serious note and interesting fact I suppose, a lot of stuff that could make a girl's personality change or act out as such alike to what one would call possessed by a demon when they don't know science is often due to abuse and for sure trauma somewhere.
So one may think she peed on the floor must be a demon within her. No more likely she experienced something traumatic. Which is why doctors and therapist would be best choice to find the route of the problem.
Also in the exorcist when it's blamed on it been the demon, in reality it could simply been her doing it because she acted out due to not getting the affection and attention she needed. Humans are nothing more than animals and what do cats or dogs do when there not getting as such the act out by doing this too.
So the answer will always be simply found by something close to home. Whether it is abuse or neglect.
