I could be wrong. Maybe the necklace is highlighting Ivana's Adam's apple and the muscularity which makes her neck about twice the thickness of her husband's neck. Maybe the bigger breasts make the other appear more feminine. Didn't think of it.

That bump is not just a trachea, trachea would stick out at the bottom portion of the neck slightly more than at the top. For sure there would be a line, not just a ball.
