Why is she suddenly acting nervous?

There is a girl that I have seen around my gym for a few month now. She always makes prolonged eye contact with me and will occasionally smile at me when she walks by, although we have never actually talked. One day as we were walking past each other I gave her a smile and wave to be nice. Since then she has been acting nervous and avoiding eye contact when we are near each other. I occasionally still see her glancing at me when she isn't near me. I was just trying to be friendly and now I feel like I creep her out.

So most likely she was trying to get your attention with the stares, smiles and then waves. It is probably the reaction you gave or lack thereof that made her stop. I take it that over the months she was doing this you never approached her. That is what I get from the details you provided. So... she gave up thinking you weren't interested. Ask yourself how long you would keep doing that to someone because you are interested in them and not get anything in return? Eventually you would tire of trying to get someone to ask you out or whatever and quit and move on. That is most likely what happened here. You just let that go on too long and she gave up.
