Why would boyfriend have extreme reactions to his ex?

We live in a small town and my boyfriend and I have occasionally run into his ex. When we do, he freaks out and acts weird.

Example: we walk into a cafe and he suddenly freaks out, turns around, runs into the wall, hurts his arm and then walks out the door- leaving me inside. I turn and see a girl who I later learned was his ex.

Another time, we went out and parked the car on the street. We get out and begin to walk in. All of a sudden he wants to re park the car, on the other side of the restaurant. I look over and see his ex across the street. She didn't even notice him. She was playing with her dog. As soon as he heard her, he acts weird.

Another time, we passed her office and he drove so fast to pass it, then slowed down.

I thought he was scared of her cause she did something but he said no so I don't know why he acts so weird around her.

It's been maybe 1.5-2 years since they last spoke.
