Sure. They react as you would expect when a man walks in accidentally.
I think I did it once in an airport. The signs weren't the standard signs and the entrance wasn't a door. The room was huge and angled around a medium wall. I was waiting for a two-door set up. A man was walking out at that stage so I never passed it.

No and no. You just have to be careful at those restaurants where they “uniquely” name their restrooms. For example, I know this chicken that the ladies’ room is labeled “Hens” and the men’s room is labeled “Roosters.” You better know the gender of animals. 😅
