Assuming there aren’t any conspicuous distractions then the back room of my brain will run a diagnostic analyzing her chakra. That will give a general outline of her overal character, and if she should be avoided or not. The main controls (my eyes and front section of my brain) focus on the physical activity like tiny muscles in her face, eyes, attitude, and voice tone/frequency. This scanning process is not limited to female humans however. I apply it to all sentient beings.

It depends.  If I'm standing or walking then usually her eyes. If I'm in my wheelchair the her boobs are generally at eye level for me...

It really depends on how I see her first. Different girls present themselves differently, and a girl who has one feature that really stands out might be noticed for that first, but another one would be noted for something completely different.
