My opinions are
1. take him outside and make a surprise party if u have some money.
2. Try candle night dating and make it a special day that u will never forget. Chose a place with less lighting and which can create a romantic ambience!. U two can enjoy a lot.
3. U can take him for a movie and be go around to some beautiful places and u can have a dinner together at night and it will be a great day for sure.
4. out of points, lol!.
Don't forget to buy him a simple gift and it will be a valuable gift that he can treasure it!. Gift is necessary and it has to be a small gift. Bday gifts cannot be avoided for sure. I think that u know him very well and u know what he needs the most at this time. Chose a gift that is he is trying to get for a long time and he couldn't get it because he busy around with living this life!
There r lots of Bdays coming. U have to make some variety of plans for that too!
I'm not one to give "huge" surprises, personally I always appreciate the thought rather than the present itself. The best feeling is that someone thought about me.

lol no balloons please. Well that depends on his interests. If he's a gamer get him a cool game, a reader get him a good book. Or a pair of cool Nikes will also rock lol!
sex would be best but if you are close enough know many things about him then he surely would have told you things he liked then remember that and give him a good materialistic gift
take him out for a dinner at night somewhere nice and calm and maybe try to find what he likes and get him something small.
Have a lovely conversation that's just enough have a good open talk make him fell luck to have u in his life
