What would be your biggest fear if you suddenly shrunk down to the size of a ladybug?

Freezing to death. We have a very slow metabolism relative to size. At that size. You'd probably be dead in a few minutes. Because you wouldn't generate enough heat.

Drowning would also be a major issue. A drop of water could kill you due to surface tension and we lack a superhydraphobic coating.

Insects and bad smells. I heard somebody in Toronto was poisoning skunks which is illegal but I think they did it cause they hate the smell.

None. At that size a human would have the mental capacity of slug. Unless all cells were scaled, in which case respiration would be impossible. Dead quickly, if not instantly, either way.
