I would normally wait for them to be amazed by the prank and swallow it completely. Then tell them "April Fools!" so that we could both have a good laugh. I wouldn't want them to believe it for long or it might make them feel stupid.

If I told a group of gullible girls that it was World Penis Appreciation Day and they agreed to do whatever I asked, I would never admit that it was a prank. But wait a minute! What if the knew I was lying, pretended to be gullible, and agreed to do whatever I wanted as a prank on me?

I guess it depends what the joke is.
One time at work, I recorded by coworker at another kiosk signing off, then played it over the radio the next day. He must've been so fucking confused. I don't know if he figured out it was me, but I never told him.
And another time, my co-worker came in and I pretended like someone called me asking details about the murder we had, and I was like, "No, they haven't found the guy yet. No there's no immediate danger to the public. Well, that's something you'll have to talk to the rangers about."
My coworker totally thought it was a real phone call, and I have no inention of ever telling her its fake.
Sometimes I watch those hidden camera shows and I'm like, if I did that, I just wouldn't say anything and let the person be confused for the rest of their lives
April fool's jokes are cruel and unethical. Here's an example of one that went horribly wrong and ended up in the death of innocent pufferfish:
