I feel like am at the end of the road with my life at 38, what should I do?

Im 38 and not good looking at all with no girl and no kids. I never had a real girlfriend and never been intimate with one unless i paid for it. I have multiple sclerosis which affected my sex life and have erectile dysfunction; I’m not very big either (6.5”). I have a city job and live with roommates. I feel like at 38 there's nothing more for me than just to be miserable and work.

I'm not older than you but i don't think 38 is a lot actually and i think there is so much about life that even 100 is not enaugh. A lot of people have felt at least once the same as you do. I think you should start working on yourself, make yourself feel better, think of it as a puzzle that you have to start putting piece by piece.
I remember feeling like shit being depressed and always stressed, never managing to gain weight (looked like a sceleton and always sick and pale) that added even more stress and low selfesteem until i managed to "pick myself up" and do something because i was getting tired of feeling like that. Start with healthy diet, start working out your body is going to change, your face is going to change, your health and your life. Erectile dysfunction i don't know the cause but if you do and i think most of the time it is fixable. Scoliosis i feel you, i had it in two places even tho it was mild i fixxed it with lifting weights (swimming helps a lot). Maintaining yourself is a never ending game. You change your mind you change your energy and start attracting more people.

Well 6.5 inches is not small but you’re still young and can still get a girlfriend, they don’t always go by looks , personality is a big factor

When you say you never had a real girlfriend are you saying that you did date someone previously but it was not a true relationship?

Last girlfriend i had was in i only had girlfriends before high school but only like 3 and it only lasted months and never had sex with them
