Yes it's possible. Cheetahs are a good example of a species with a limited gene pool that makes them vulnerable to diseases. The less genetic variation in a population the more at risk it is and that includes humans. Even in the west you'll find pockets of people who look similar due to inbreeding and certain diseases can be more prevalent in certain areas.
So it would be possible for an isolated group to become extinct but as a whole we move around too much for that to happen
