Hypothetical: A white man is bleeding out on the street. A gay trans woman is coughing up blood. Who should be helped first?

According to woke FEMA'a equity policy, the gay trans woman would recieve help first because they're apparently oppressed. 🤷🏻‍♂️

If both are in the same situation -- that's for triage to determine. The same first responders in the same place should not -- and those who I know would not -- care one whit about the socioeconomic status, identity, political views, and so forth -- take any of that into account.

But if there's a shortage of resources (have you ever seen an ER, where there just aren't enough people to help everyone?), the order of treatment will depend upon triage. The idea of triage is to focus on the people who will recover with treatment. Those who are in such bad condition that they have no hope of survival, even with intervention, are given palliative care for comfort, while those who will recover on their own or whose cases are less urgent will receive treatment as time allows. It's those in between, whose lives depend upon prompt intervention, who will receive prompt treatment.

So if you present at an ER with chest pains, or stroke symptoms, you'll get seen right away. Time is of the utmost essence in case of a heart attack or stroke. If you have a broken arm, you're likely to be waiting around for quite a while, as there just isn't the same kind of urgency.

Now, there's the issue of distribution of resources -- wealthy communities tend to have easier access to good hospitals and faster first response -- but no ethical first responder would take anything other than medical need into account.

As far as mass casualty goes, that is defined by more victims than you have resources for, if they are close to dying or will probably die despite your interventions, you move to someone that can be saved.
If you can easily control the bleeding you do that, the move on to the next victim.
Obviously if someone is caughing up blood that indicates an internal issue, so for them it would be bright lights and cold steel.
Depending on the amount of fluid loss they also might need the same treatment, so their vitals would tell you more.
If they are stable then direct pressure/tourniquet may help unless they are hypovalimic, then other than a fluid bolis there isn't much else you can do except try to controlthe bleeding,, and place the other patient on their left side so they are less likely to aspirate any of the fluids i to their lungs and wait for the ambo to come.

This is a gross misinterpretation of what is going on. Of all the medical personnel I know, they are trained then, now, and always in triage situations, to treat those closest to death, first. You have a minor cut, you're back of the line. You're having a heart attack, you're front of the line regardless of who you are. This will never change.

It is a known fact that white wealthy communities in disasters get far more care then those that are not. Please for goodness sake, google it, and don't take this as "oh another woke/racist person". This and other meetings like it are designed to address that disparity in giving aid to what are considered disadvantaged communities to be on par with those that are not. Take Covid for example. My friend was on the task force with the white house, and part of their plan included more boots on the ground in poorer communities. Why, because they had less access to transport to go get a covid shot, or go see a doctor. There is a reason organizations before disasters now, hit up homeless communities and those living on the street---because previously they were left for dead and uninformed, now thanks to discussions like these, disadvantaged groups are given the opportunities for shelter and aid.

The white man should be helped first since - more likely than not if he is bleeding out it is a life or death scenario.

The trans person who is coughing up blood may or may not be in a life or death situation at that moment but it is also possible that it may be due to some disease or condition that - although requires urgent attention - may not be a a life or death scenario of the moment.

Need more context to the bleeding out. Is it by an Arterie or a flesh wound? Coughing blood would suggest internal bleeding. by the nature of organs have more blood flow.

This is why triage is a thing and has been improving by trial and error for close to a hundred years now. Triage and you can look at the. gov step by step analysis dosen't give a fuck who the patient is as a person just the medical necessity to sustain life from most extreme to less as effective as possible.

As a future medical student, my thoughts would be to help both.

But in particular I would help the man bleeding in the street first, coughing up blood is not always a serious emergency, sputum with blood is a fairly common occurrence during infection of the lung area.

The man bleeding out can be helped with a bandage, the person coughing up blood with either survive without help or needs professional medical help to address internal injuries for which few people are qualified.

So clearly you should devote your efforts to the man you can save, and is certainly going to die if you don't help.
