For those of you that live in other countries (not the USA), how do you address your teachers when speaking to them?

In the US, it's customary to call them Mr. Smith or Miss/Mrs. Smith, I was told today that other countries call them by their first names. Is this true?

In school always Herr/Frau and last name. In university it’s possible a (younger) professor tells his students to call him by his first name, but I saw that only once.

Over here in the Netherlands a lot of elementary school teachers work on first name basis. With a few exceptions.

We've always just called our teacher by first name.
