I've always been a hardcore socialist so best life possible for everyone unconstitutional as a minimal is the bottom. We can and should argue what that level should be but not the point here.

I've also warmed up to Euthanasia. Being of the highest ethical socialist life for everyone isn't enough for some with all Avenues ethically explored like being paralyzed from the neck down. While euthanasia is a disappointing and an example of failure of human ingenuity it is an ethical outcome.

So to formally answer your question. The world we live in. We can do so much better and we actively hate even prey upon others.

Death of a child or my partner. Especially a premature one.

I remember seeing this thing about a woman who took a job in Japan. A guy kidnapped her, raped, tortured and murdered her and she suffered so much before she died. They guy got out because the family paid restitution to the father. Naturally the mother divorced her husband after that. But anyway losing my child and having the knowledge of how she died and the guy got away with it would be worse than anything

Being unable to even get out of bed. I wasn't paralysed, but for about a month I couldn't really walk, because it was just far too painful to even move. Thankfully I was able to have an operation to get the problem fixed (because we have free healthcare in Australia).

Having been temporarily paralyzed by Guillian Barre Syndrome I share that fear but worse than death? Nah. I'm more scared of outliving my money or existing in poor health. I've never been scared of death. It's just silly!!

Alzheimer's or dementia. If your heart is still beating but your consciousness is gone, are you still you? Are you still alive? Do you wanna go out in a dirty diaper drooling on yourself with no awareness of where or who you are?

Vertigo is a close second. They kind where your balance is severely off even laying down with eyes closed. I could not handle that it was awful.
