It honestly depends. If the person knows about that person's illness and is trying to say that they are mentally ill because of their illness then no. But if the person does not know whether the person has a mental illness and just saying it to be name-calling then yes, it's an insult.

If it is untrue, it is a terrible insult and also very stupid, particularly if the person who said it is unqualified to diagnose mental illness - which comprises the majority of people who make such comments.
