A milking goat. A small barn. And environment-apporpriate shoes.

That's the best I can think of with 3 things. This is harder to do when I don't know if it's gonna be cold, humid, windy, jungle, barren, caves, swamp, etc. It's hard to know what will be there and what I need to bring.

But I definitely need a place to keep the goat and myself (will probably need shelter way sooner than I can build it).
A goat for milk (or meat) because I want a little padding against dying of starvation or dehydration until I figure out how (or if I can) get food/water from the island.
And shoes because no matter what kind of island it is, I'm gonna need shoes more than any other clothing item (& probably way sooner than I can make them).
