Obviously, you both put your best foot forward. Impeccable hygiene is a must for both. If he's picking her up in a vehicle, it should be totally clean too. He should pick her up on time, she should be ready on time, and she shouldn't make a fuss about who pays for the food or drink if that's part of the date.

And he shouldn't make a fuss if she orders many expensive foods or drinks. He should obey and serve her. If he doesn't have enough money then he should work as a waiter for free as long as he pays the bill fully.

Yours is a very odd response. Let me give you few things to think about.
1) If a man offers to take a woman to an expensive restaurant on their very first date, then money isn't an object to him and he doesn't care what she orders. So, not having enough money isn't in play here. However,
2) A lady with class, REAL class, will never order the most expensive things on the menu simply because she can. Oh yes, I've heard women say "it's a test" for him, and all that BS. Don't forget, it's a test for YOU too.
3) No man with any class would take a first date anywhere that he couldn't fully pay for.

If a guy asks a woman out to dinner, and the conversation goes like "um, I'd rather go to a night club if that's ok with you", it's already awkward to me. What in the world does "happens to be a good girl", even mean, or have to do with anything in this scenario?

LOL You think you have that kind of clout? Your colloquialisms do not offend me, but rather, I often don't understand them so I ask for clarification. Once again, I see zero correlation between what type of girl you are and first date courtesy. I presume that if a guy is asking a known slut out on a date, he's not going to care very much about chivalry OR what she wants, unless it would help get him laid.
