Because unless they are watching, there's nothing to call their attention to the first punch. The commotion following the first punch but immediately preceding the second punch gets their attention and they pay attention in time to see the second punch.

Exactly this. And the smart teachers have informants that will tell them the truth of what actually happened later in the day so discipline is more fair.
