How do they make sex scenes in movies and TV shows look so real?

Mostly just serious acting between two people comfortable with each other. Usually there's a reduced crew for that kinda scenes to make the actors more comfortable with their surroundings. Camera angles also help, and if the actor doesn't want to do those scenes, they use body doubles with digital face replacement if they are clearly showing faces.

Though it's sometimes happens that the actors are doing it for real. The first that comes to mind is Original Sin. I don't actually know if Banderas and Angelina Jolie were having actual sex in that scene but it was rather... Convincing.

Simple, they make it almost real, the actors have a lot of sexual experience and just replicate having sex or intense making out as they would with their actual partner just with underwear on and without actually doing it, some movies have them actually have sex, which makes it actually real.

They know what you like to see. People with loads of sex experience know what to do and what makes you happy to see. The most important detail is arousing the viewers with erotic movements. Such as caressing and french kissing.
