Wow, "I Spit on Your Grave" is definitely intense! One movie that comes to mind when talking about messed up films is "A Serbian Film." It's notoriously graphic and disturbing. Another one is "Requiem for a Dream." The descent into addiction is portrayed in such a raw, gut-wrenching manner. You'll find yourself thinking about these movies long after the credits roll.
On a different note, "Oldboy" from South Korea is a mind-bender that combines psychological torment with jaw-dropping plot twists. Definitely not for the faint-hearted but brilliantly made! 🤯🎥

In 2004, the Library of Congress added "Eraserhead" to the National Film Registry because it is "culturally, historically, or aesthetically significant" (those words come from the legislation that created the NFR).
There is nothing like "Eraserhead" and how the film got its name is a bit disturbing.
It's a surreal nightmare. You may not be able to get through a first-viewing... or even a second-viewing... but eventually you will and you should see it. It will change you forever and you will then have your permanent answer to this question.By the way, the central character is Henry; he's the "eraserhead" for an obvious visual reason but, later, for a reason you will see that will then be obvious.

With hindsight the film seems mostly therapeutic, considering the metaphor about fatherhood. That being said, I watched 15mn only and then we left the cinema room with the girl I was more or less trying to date.

And I don't care much about messed up things. Of course on a personal level it will never reach what I consider messed up more in the psychological stuff like Cam. But if your talking messed up as extreme what they show ABCs for sure.
There is more extreme stuff but I don't know I feel like I can watch that stuff and shrug.

I'm sure there are some that are even more messed up, like "Serbian Horror Film", which I was told not to watch. But the most messed up one I'VE seen was "Tusk". My co-worker put it on, and I was actually into it at first, but I made him turn it off as soon as he started turning the guy into a walrus

Yeah, don't watch it. I made it him turn it off before it got to the really disturbing parts, but I read the plot. Absolutely not
Anonymous(25-29)1 yI don't remember the name of it. Let's call the girl Kayla and the guy Pete. Kayla was raped and her rapist was never caught. She would accuse random men of being her rapist because she would swear their voices or their hands were the same voice/hands.
Her sister brought home her date Pete and kayla again swore he was her rapist (he wasn't) She held him hostage, mentally a d physically tortured him, then hired to male prostitutes and had them rape him (she told him he had a rape fantasy)
Pete escapes at the end and ends up raping Kayla to get revenge. Left me feeling very conflicted. I felt bad for both of them.

1 yXanadu helped birth The Razzies, the annual awards to the worst in entertainment. It's kind of unfortunate. Xanadu music was good but the movie was horrible.
