I usually breathe in through my mouth and out through my nose and if I can't breathe fast enough that way, I'll exhale through my mouth every so often on her neck or something. A lot of girls seem to think that's really hot. I don't know how I'd feel if a girl did that to me.

Just breathe any way you can and don't hold your breath longer than you need to. I don't think there is a rule behind this, other than humans need to breathe to stay conscious.
